Continuity of Care sebagai Strategi Pendampingan Ibu dan Bayi: Studi Kasus Asuhan Kebidanan

This case study aimed to describe the implementation of continuity of midwifery care for Mrs. D, a 26-year-old primigravida, from the third trimester of pregnancy to family planning services at Shanty Clinic, Medan. This study used a descriptive case study design based on Varney’s seven-step midwifery management and SOAP documentation. Data were collected through history taking, observation, physical examination, labor monitoring, and documentation review. The findings showed that pregnancy progressed physiologically with minor complaints, including frequent urination and fatigue. Labor occurred spontaneously; a male newborn was delivered with a birth weight of 3,250 grams and a length of 48 cm, with good general condition and early breastfeeding initiation. Postpartum care was conducted through repeated visits; fatigue and breast discomfort improved after counseling on breastfeeding techniques, nutrition, rest, and postpartum exercise. The newborn showed no danger signs during neonatal follow-up. At the end of the postpartum period, the mother chose the Lactational Amenorrhea Method after receiving counseling. Continuity of care supported early detection, health education, and comprehensive monitoring of the mother and newborn.

Treatment with a different view of health. Perineal wound infection is one of the causes of maternal death with a proportion of 20-30% and 25-55%. Objective: This study aims to find out how perineal wound care is carried out based on the views of the Javanese tribe in Tanjung Rejo Village in an effort to maintain maternal health after childbirth. Methods: This study uses qualitative methods with an ethnographic approach, the research sample was obtained through snowball sampling. Data collection was carried out by in-depth interviews. The research informants were the main informants of 7 postpartum mothers and supporting informants namely, 1 traditional birth attendant, 1 midwife, and 1 community figure. Results: The results of this study indicate that Javanese people in Tanjung Rejo Village still treat perineal wounds based on Javanese customs (culture). The treatments carried out are, pilis, parem, drinking herbal medicine, boiled water from betel leaves, octopus, bengung, walikdadah massage, sitting senden, prohibitions and abstinence from eating which are believed to be for the health of postpartum mothers and babies. This treatment has various impacts, namely, the positive impact (benefits) can speed up the healing of perineal wounds and warm the mother's body after childbirth. The negative impact of using octopus, bengkung, walikdadah massage can cause bleeding and discomfort to the mother in caring for perineal wounds. Abstinence from eating also causes perineal wounds to take a long time to heal and causes a lack of milk production and malnutrition. Conclusion: Society must be good at choosing good care for postpartum mothers. And the community carries out treatment according to the recommendations of the health team. Health workers or midwives are expected to provide good education to the community.

Family-related factors, particularly maternal knowledge, attitude, and stimulation practices, are important for early prevention and management. Objective: This study aimed to analyze the relationship between maternal knowledge, attitude, and stimulation with speech delay among children in the working area of Medan Labuhan Public Health Center. Methods: This analytic quantitative study used a cross-sectional design. A total of 35 mothers who met the inclusion criteria were selected using purposive sampling. Data were collected using questionnaires and observation of children’s speech development, then analyzed using univariate and bivariate analyses with the Chi-Square test at a significance level of 0.05. Results: Fourteen children (40.0%) had speech delay, while 21 children (60.0%) were categorized as normal. Maternal knowledge was significantly associated with speech delay (p = 0.012; OR = 8.00; 95% CI: 1.73–37.09), as were maternal attitude (p = 0.030; OR = 5.76; 95% CI: 1.31–25.42) and maternal stimulation (p = 0.001; OR = 34.83; 95% CI: 5.02–241.72). Conclusion: Maternal knowledge, attitude, and stimulation were significantly associated with speech delay in children. Maternal stimulation showed the strongest association, highlighting the importance of family education and early language-development screening in primary healthcare settings.

Postpartum mothers' perceptions of umbilical cord care are influenced by knowledge, cultural beliefs, experiences, and sources of information received. Methods: This study used a qualitative method with a phenomenological approach using the snowball sampling technique. Data collection was carried out through in-depth interviews. The research informants were 5 postpartum mothers in caring for the umbilical cord. Results: Four main themes were found, namely: mothers' understanding of umbilical cord care still varies, some are in accordance with medical recommendations but others are still less appropriate; cultural beliefs still influence care practices, including the use of traditional ingredients such as turmeric, coconut oil, and powder; mothers' knowledge of signs of umbilical cord infection is mostly limited to mild symptoms, without understanding the risk of systemic complications; mothers' sources of information come from health workers, KIA books, social media, and family and friends, with the dominance of information from families that sometimes contradicts medical standards.

This study aims to explore the perceptions of pregnant women regarding high-risk pregnancy at Tanjung Rejo Public Health Center, Percut Sei Tuan. Method : This qualitative study used a phenomenological approach, involving five pregnant women identified as high-risk. Data were collected through in-depth interviews and analyzed using data reduction, presentation, and verification methods. Results : The results show that pregnant women’s perceptions of high-risk pregnancy are influenced by personal knowledge, previous experiences, and information sources from healthcare providers and family. They recognize the importance of regular check-ups and preventive measures, yet some experience anxiety and uncertainty about potential risks. This study is expected to serve as a basis for healthcare providers to offer more effective education and psychological support for high-risk pregnant women.

Relactation serves as a solution to restore milk production, yet it requires a systematic and collaborative approach. Objective: This study aims to describe the success of relactation through a collaborative approach with oxytocin massage intervention in postpartum mothers. Methods: This descriptive case study with a prospective approach was conducted on a postpartum mother on day 1 (Mrs. L, 31 years old) experiencing inadequate milk production and formula dependency. The intervention was conducted for 7 consecutive days, including breastfeeding technique education, emotional support, and oxytocin massage for 15–20 minutes. Data were collected through in-depth interviews, direct observation, and daily progress tables. Results: On the first day, milk production was low, breastfeeding frequency was 5 times, and formula use was 6 times. Following regular collaborative intervention and oxytocin massage, a significant increase in milk production occurred. By day 7, breastfeeding frequency increased to 10–12 times, formula use completely ceased (0 times), and the mother felt more confident and motivated. Conclusion: A collaborative approach combining lactation education, family support, and oxytocin massage intervention is effective in increasing milk production and the success of relactation in postpartum mothers.

The community health center with the lowest triple elimination coverage in 2022 was Bangko Community Health Center, with 68% of the 100% target coverage. Factors suspected of being associated with triple elimination screening in pregnant women include perception, family support, and the role of health workers. The purpose of this study was to determine factors associated with triple elimination screening in pregnant women in Kungkai Village, within the Bangko Community Health Center, Merangin Regency. The study used an analytical survey with a cross-sectional design. The study was conducted at the Village Health Post (Poskesdes) during a pregnancy class in Kungkai Village, within the Bangko Community Health Center, Merangin Regency, in 2024. The population in this study was all pregnant women in their first to third trimesters in Kungkai Village, within the Bangko Community Health Center, Merangin Regency, during the period February/June 2024, totaling 32 women. Because the population was only 32 individuals, the entire population was selected as a sample. Data were collected through interviews using a questionnaire. Data were analyzed univariately and bivariately using the chi-square test at a 95% confidence level (p-value ≤ 0.05). The results showed a significant relationship between all independent variables (perception, family support, and the role of health workers) and triple elimination screening in pregnant women. It can be concluded that perception, family support, and the role of health workers are factors associated with triple elimination screening in pregnant women.
